I've had a busy schedule so I've only been able to glance at this :(
The way I've set up Fate in my campaign is the notion that the most selfish thing one can do is self-destruct. It's entirely self-centered, and it ends badly for you too.
>From there I scale it--what kind of self-destruction can this character do that will take the most possible loved ones down with him? Most possible people in general? I take circumstances into consideration--an old lady who lives by herself won't have the reach of a jetsetter with influence.
I had to jot down Destiny/Fate for the major characters in the story, and six-scale them for check digits (to avoid too much spoilerage) for when we had a Master of Destiny on board. Sadly we don't have one now :(

Destiny is a little trickier--it has to be both a stellar accomplishment, but it also has to be selflessly. Climbing Everest isn't a Destiny. Saving some poor sap from freezing to death up there, is.
