Rather depends on how many people take the deal.  This or that
individual might take ten years of good stuff and a quick death over a
longer life of relative poverty, but society as a whole may look on
that option with contempt, horror, or simple lack of interest.  We
might liken it to, say, quasi-legal use of harder drugs in Holland.
People *can* get it, but most choose not to.
