My first thought would be to have the commander killed. Maybe by
wildfire but it would be great to have him walking through the yard,
talking with the PC about who he thinks the traitor is, or telling them
about something equally important; when a stray arrow takes him in the
neck.

Very George R. R. Martin-like.

It would then be interesting to see who got picked as the new leader of
the keep. Obviously the PC sub commander is the best choice, but
(especially in GRRM's world) that doesn't mean that he will be. Imagine
if some nobleman's son from a better family gets the job and decides
that he is going to take the men on the offensive to show up the sub
commander.

Other then that any type of mutiny scenario sounds like fun.

Imagine if the defenders broke up into factions, each one controlling a
portion of the keep. None of them want the besiegers to break in, but
they all want to see the other factions within the keep defeated. 

Maybe this could even be the spy's plan? t-l
