Plague breaks out (if cured by divine magic, maybe the people do not get
immunity and can catch it again)

Some (unknown to the party) wizard opens a portal and nasty little imps come
out

A noble falls in love with a character and I am sure you can plot out issues
there

A NPC starts having visions that causes the party to take certain actions.
Whether the visions are true or the person is insane is up to you

The rats in the keep are intelligent and have some basic magic skills

Internal conflicts among factions of the defenders.

Why was that strong hold built?  Who Built it?

Perhaps there are secret passages and tunnels in and
out.

Maybe there is a hidden artifact related to whole
other plot and trasure hunters causing havok trying to
get at it.
