It's called "Only the Best Shall Win", and is in CII, IIRC.

I tried it, and didn't like the effect it had. One of the main things
I noticed was that characters ended up only using aimed attacks on
mooks with poor defences, and vs good opponents they stuck with torso
attacks because they couldn't afford the penalties. The result was
fights that were the exact opposite of what I wanted.

I later realised that this was because OTBSW penalises attacks with
'to hit' penalties twice, once because it's harder to get the hit in
the first place, and then again because the hits you do get are easier
to defend against.
